Consent to Participate in Church Activities and Events
My child has my permission to participate in White Oak Kids (the “Ministry”)’s activities and events offered as part of the Ministry’s [Youth/Children’s] program during the 2026-2027 school year, and September 1, 2026 and August 31, 2027, including regular on-site ministry programming (such as routine classroom activities, supervised recreation, chapel, and other similar activities), as well as transportation provided by Ministry staff and/or volunteers to the limited, low-risk off-campus locations specifically identified below. This annual consent ONLY covers:
· regular on-site ministry programming (e.g., classroom instruction, supervised recess or play, indoor/outdoor games comparable to school recess);
· the following off-site activities and, if applicable, transportation by Ministry staff or volunteers to these activities:
o [e.g., supervised walking trips on or immediately adjacent to Ministry property]
o [e.g., routine supervised walks on a designated neighborhood route]
o [other low-risk, off-site activities such as a Bible study and/or game day at a local park]
No other off-campus locations or transportation are covered by this annual form. All other off-campus activities—including activities requiring vehicle transportation to any unlisted location, public events, recreational venues, water activities, larger community parks, or locations with elevated or unpredictable risks— require a separate event-specific consent and release.
Medical Treatment
I give permission to have my child treated in case of medical emergency. In the event of a medical emergency and I cannot be reached, I hereby authorize the Ministry’s staff or volunteers, and/or emergency and medical personnel to make emergency medical decisions for my child. I acknowledge that the Ministry does not provide any health insurance covering my child the event referred to herein, and I further acknowledge that it is my responsibility to obtain health insurance covering said child. I agree to accept the sole responsibility for the cost of medical care, including emergency transportation if deemed necessary by the Ministry.
Photographs/Recordings
I also grant permission to the Ministry, and its representatives, contractors, employees and volunteers acting on behalf of the Ministry, to take and/or use, copyright, publish, edit, crop or treat images or likenesses of me or my child(ren), including photographs, videos or otherwise, for any lawful use on the Ministry’s website, social media pages, blogs, or in other official Ministry printed or electronic publications without further consideration. I understand that the consent and release for this section will operate in full force and effect until I revoke my consent in writing. I understand that should photographs or videos of me or my child(ren) be used on Ministry-owned or operated websites or webpages, they may be available for download.
Assumption of Risk/Indemnification/Hold Harmless/Liability Waiver
I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL THE RISKS WHICH MAY BE ENCOUNTERED DURING THE EVENTS AND ACTIVITIES ANTICIPATED BY THIS FORM AND THE TRANSPORTATION THAT WILL BE PROVIDED BY MINISTRY STAFF AND/OR VOLUNTEERS TO SAID EVENTS AND ACTIVITIES. In consideration of my child being permitted to participate in said events and activities and other valuable considerations the receipt of which is acknowledged, I HEREBY AGREE TO RELEASE, DEFEND, INDEMNIFY, AND HOLD HARMLESS the Ministry, its employees, volunteers, and other agents, from any and all past, present, and future, known and unknown liabilities, causes of action, claims, expenses, personal injuries, and damages, INCLUDING THOSE CAUSED BY THE NEGLIGENCE OR FAULT OF THE MINISTRY, ITS EMPLOYEES, VOLUNTEERS, OR OTHER AGENTS, and including without limitation, interest, penalties, court costs, attorney's fees and expenses resulting from or on account of injury to my child, myself, or my or my child’s property in connection with any transportation or event or activity anticipated by this form.
I EXPRESSLY AGREE that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted in the Commonwealth of Kentucky and that if any portion hereof is held invalid, it is agreed that the remainder shall, notwithstanding, continue in full legal force and effect.
I ALSO AGREE that any dispute related to or arising out of this document or my child’s participation in any activity or event anticipated by this document will not be taken to court, except if required by law or to enforce an arbitration award. Disputes will first be addressed through good-faith discussions (up to 30 days), then through Christian mediation with the National Center for Life and Liberty or another agreed mediator. If mediation fails, the matter will go to final, binding Christian arbitration, or if no arbitrator is agreed upon, to the American Arbitration Association. The arbitrator’s decision will be final, and I EXPRESSLY WAIVE THE RIGHT TO SUE THE MINISTRY IN CIVIL COURT EXCEPT AS ALLOWED ABOVE.I have carefully read and understand the foregoing, and I sign this legally binding document as my own free act.
